Cortisone injection: medical questionnaire and consent

Complete the cortisone medical questionnaire and read the risks. Your answers will be submitted for clinical review; the £50 prescription-fee link is sent later by the clinic if appropriate. No prescription fee applies to the £95 (subject to availability) assessment and scan.

After submission, the clinic will review your questionnaire and contact you. The £50 payment link will be sent only if appropriate. Do not pay before clinical review.

1. Patient and treatment details

2. Medical safety questionnaire

Please answer every question. A Yes response does not automatically exclude treatment; it requires individual clinical review.

Have you had an allergic reaction to corticosteroids, local anaesthetic or any injection ingredient?
Do you have diabetes or problems with blood glucose control?
Do you take blood thinners or have a bleeding disorder?
Are you immunosuppressed, on immunosuppressant medication or undergoing cancer treatment?
Are you pregnant, possibly pregnant or breastfeeding?
Have you had joint replacement or recent surgery in the proposed area?
Have you had a steroid injection recently or a previous injection complication?
Have you had a recent vaccination or do you have one planned soon?
Do you have another medical condition or concern the clinician should know about?

3. Corticosteroid injection: information and consent

The intended purpose is to reduce inflammation and pain, potentially making rehabilitation easier. Benefits vary and may be temporary; treatment may not help. Other options can include assessment, exercise/physiotherapy, activity modification and discussion of other medical care.

Risks and limitations: temporary post-injection pain flare, bruising or bleeding, skin colour change or fat atrophy, temporary raised blood glucose, facial flushing, allergic reaction, infection (rare but potentially serious), and possible tendon or tissue injury. Repeated injections may have additional risks. Your clinician will explain risks specific to the site and your medical history.

Aftercare: seek urgent medical attention for increasing redness, heat, swelling, fever or severe worsening pain after injection. Follow individual post-injection advice.
