This page combines the pre-injection medical questionnaire, corticosteroid consent, anti-inflammatory / stomach-ulcer risk consent, and a post-payment prescription workflow. Forms can be completed online with typed-name signature and drawn signature pad. Submission is designed to happen only after successful payment.
If you are taking anti-inflammatory medication, combining this with steroid treatment may increase the risk of stomach irritation or stomach ulcer problems in some patients. Gastro-protective medication may be needed where clinically appropriate.
After payment, the completed forms can be sent to info@wellbeingandhealthphysio.co.uk and amanda.mcdermott@covwarkpt.nhs.co.uk. The prescription summary is preconfigured with the requested medicines and quantity, with the client number omitted.
