Start with the real goalRelief alone is not always enough if the tissue still cannot tolerate normal life and sport. When physiotherapy should usually leadMany tendon, overload and stiffness problems still respond best to a clear exercise and load-management plan. When injection can helpGuided injection can create a more workable rehabilitation window in some cases. How we make the decisionDiagnosis-led care beats algorithm-led care.
Start with the real goal
Injection can reduce pain, but it does not automatically rebuild strength, mobility or confidence. Physiotherapy is what usually restores those capacities. That is why we normally frame the decision as rehab alone versus rehab plus injection, not rehab versus injection.
When physiotherapy should usually lead
- Symptoms are manageable enough to exercise.
- The main problem is weakness, stiffness or deconditioning.
- The patient can progress without needing rapid pain suppression.
When injection can help
- Night pain or high irritability is blocking progress.
- A clearly defined inflammatory target is present.
- Previous rehab stalled because pain remained too reactive.
How we make the decision
We use clinical assessment and ultrasound where appropriate to decide whether the next best step is rehab, guided injection or a staged combination of both.

If pain is affecting sleep, walking, sport, work or confidence, the next step is usually a clear diagnosis rather than more guesswork.
